You can continue to use the same gamertag if you subscribe to xbox live after your trial has expired. the date on the back is probably the time it expires, so just check your email when it does and you can renew it with a subscription and keep your gamertag

also if like you say you left xbox live for a while, and you'd like use that same gamertag again, you can use the account recovery feature to get it back.

If You Have Xbox And Then You Get Xbox 360 Then You Can Use The Recover Gamertag Thing On The Xbox 360 Dashboard. I Should Know I Have A Xbox 360

also some trials work past there expiry date but not all, there is a thread about that in this forum a little ways down. check it out mang.
