If both people have seperate Xbox Live accounts, they can both use their own Xbox Live communicator when playing. Both will log in with their own accounts. If the other person, like doesn't want to use their Xbox Live account, but still wants to play, they will be called, for example, shanafanguest. If they have their communicator with them, they can still use it.

I think...but I am not sure. Pretty much, if both players have their communicators with them, they can use them.

Why buy another Xbox Live kit when you can just use the guest feature?

I have no idea if you can simultaneously play more than one account but the xbox can hold upto 7 accounts. See this page Toms hardware did a review of XBL and it states:

One thing that is important to point out here is that you only get one Gametag per Subscription Code. Additional users can play as guests, but their stats are not tracked and they don't get a Gametag. (No sub accounts might a better way of wording this, you will have to have one Xbox Live Starter Kit per user. The Xbox can handle up to seven accounts.)

If you have a seperate account on the xbox for a friend or whatever they can use their communicator but if it is just a guest off of your account they cant use the communicator if they have one of their own, but there is a tutorial on how you can use a splitter to add a headset for a friend on xb-scene, gonna do that pretty soon for my friend.

The only drawback of doing that is if you and your friend are on different teams, like on unreal for example, they would only hear the people on your team and not their team.
