every game/apps store there settings in your memory, everything is in the memory, your current health, money,... so trainers change this directly in the memory and the game uses those values
for xbox it's a litle diffrent because only one app can run but it stays the same, change the ingame values

it does apply cheats, but even if xbox had a gameshark im pretty sure it coldnt do somef te things a trainer could do, and with a trainer you can turn things off and on while your in the middle of a game

yeh u can make you own trainers, i dont know how thou, but www.xboxsaves.com is a good one to get game saves and trainers
