sounds like that. check your solderpoints.
d0 is good. otherwise the box would load it's onboard bios.

is it work when mod chip off or disconnect? if it work it could be bad flash
but if not it's could be some shot circus (try to check the every point)

The "magnified" screen is normal. Cromwell just isn't aligned..
Your problem is a bad flash.
What chip are you using and what bios are you trying to flash (name and size)?

for 1.4 or 1.5 use at least x2 4979. the 4977 doesn't have support for the focus video encoder
