What can go wrong during a softmod, and how could it be fixed?

Well if done correctly with the proper files, there should not be a problem.  If you try to rush through the process and do not know what you are doing it is very possible to screw it up.  As far as fixing them, most of the time when an xbox requires service, there will be an error code in the corner of the screen, and there is a lot of threads on diagnosing error codes.

Would I be able to use my MP3 player and hook up a USB hub to my xbox, and then use that instead of a contoler port to USB attachment?

**Edit**
I have a PAL X-Box, and if I ended up using my MP3 player, would I beable to restore it to its original setting so I can use it to listn to music?
